Guest Win/Loss Request

A Rewards Card is the 1st step in establishing the kind of personal realtionship that you'd expect from Tulalip Resort Casino. With a Rewards Card you become one of our most valued guests. Guests that are Rewards Card members are eligible to receive a win/loss statement by mail. Please click on the link at the bottom of this section and print out the attached document.

1. Please complete all Sections of the form:
   1. Year Ending, (The year you want your statement to cover).
   2. Print your full name
   3. Birth date mm/dd/yyyy
   4. Your Rewards Club number, if known.
   5. Your complete address
   6. Check how you would like to receive your Win/Loss Letter.
   7. Your signature
2. Enclose a copy of your current State Identification Card or Drivers License. We will not be able to process your request without this.
