A Rewards Club card is the first step in establishing the kind of personal relationship
that you'd expect from Tulalip Resort Casino. With a Rewards Club card you become one
of our most valued guests. Guests that are Rewards Club members are eligible to
receive a win/loss statement by mail. Please click on the link at the bottom of
this section and print out the attached document.
Win/Loss Form
- Please complete all sections of the form:
- Year ending, (The year you want your statement to cover).
- Print your full name
- Birth date mm/dd/yyyy
- Your Rewards Club card number, if known.
- Your complete address
- Check how you would like to receive your Win/Loss letter.
- Your signature
- Enclose a copy of your current state identification card or drivers license.
(We will not be able to process your request without this.)
